Program Overview

The Premier Partners Club (the “Program”) is offered by PatientFi, Inc. (“PatientFi”) to eligible Plastic Surgery, Dermatology, and Medical Spa providers currently enrolled in the PatientFi Loan Program (“Providers”). The Program provides eligible Providers the opportunity to earn rewards based on their funded loan volume or loan transaction performance for each calendar quarter in 2026.

Program Details: 

PatientFi will award the rewards described below (each, a “Reward”) to eligible Providers who meet the applicable requirements during a given quarter:

Program Requirement

$75,000 – $149,999 in net funded loan volume, or 10-19 net loan transactions in a single quarter

$150,000 – $299,999 in net funded loan volume, or 20-39 net loan transactions in a single quarter

At least $300,000 net funded loan volume or at least 40 net loan transactions in a single quarter

Cash-Back 

None

2.5% cash back on total net processing fees paid by Provider for loan transactions in the same quarter

3.5% cash back on total net processing fees paid by Provider for loan transactions in the same quarter

Email Campaign

Free professional email campaign developed and deployed by PatientFi to the Provider’s patients with approved funds available to spend, encouraging them to move forward with care before funds expire

Staff/Team Reward

$250 gift card to a vendor of the Provider’s choice

Cash Back Process:

Cash-back rewards are calculated as a rebate on the Provider’s total net processing fees for the same quarter in which eligibility is achieved. Rebates will be issued via automated clearing house (ACH) transfer to the bank account the Provider has registered with PatientFi within thirty (30) days after the end of the qualifying quarter. PatientFi reserves the right to verify eligibility and adjust or withhold any rebate in the event of cancellations, refunds, chargebacks, or other adjustments to funded loans or processing fees that occur within sixty (60) days of the end of the qualifying quarter.

Email Campaign Process:

Eligible Providers will be notified within ten (10) business days after the end of each qualifying quarter with instructions to redeem their complimentary email campaign. To fulfill this Reward, Providers must provide PatientFi with essential materials (including logo files and booking links) Within thirty (30) days of notification. Upon receipt of all required materials, PatientFi will design and deploy the email campaign on a first-come, first-served basis within sixty (60) days, sending from PatientFi to the Provider’s patients with approved funds available to spend. Failure to provide required materials within the requested timeframe may result in forfeiture of the Reward.

Staff/Team Reward Process:

Eligible Providers may select and redeem their $250 e-gift card directly within their PatientFi Provider Portal once Bronze Tier is achieved. The Provider is solely responsible for selecting the vendor and utilizing the e-gift card prior to any expiration or redemption deadlines. Any charges incurred with the selected vendor that exceed the $250 value of the e-gift card shall be the sole responsibility of the Provider. Unredeemed, lost, stolen, or expired cards will not be replaced, reissued, or credited.

Diamond Status & PatientFi Growth Fund Eligibility:

A Provider qualifies for Diamond Status by achieving Gold Tier in at least three (3) out of four (4) quarters within the same calendar year. Diamond Status Providers may become eligible to participate in the PatientFi Growth Fund, a performance-based reward program through which a Provider’s engagement and performance with PatientFi unlocks access to exclusive funds which may be redeemable for elite experiences, marketing investments, or other growth-oriented rewards, as determined by PatientFi. Eligibility for the PatientFi Growth Fund, including the availability, type, and value of any rewards or marketing investment allocations, is achievement-based and subject to PatientFi’s sole discretion. No minimum reward value is guaranteed. A Provider’s eligibility and potential allocation within the PatientFi Growth Fund may be evaluated using one or more performance and engagement metrics, which may include, but are not limited to:

  • Quarter-over-quarter funded loan volume growth,
  • Loan transaction growth,
  • Application volume and utilization growth, and
  • Overall engagement with PatientFi programs, tools, and initiatives.

PatientFi reserves the right to modify evaluation criteria, weighting, or methodologies at any time. 

Diamond Status Notification and Redemption:

PatientFi will notify qualifying Providers by email within thirty (30) days after Diamond Status is earned. Such notification will include details regarding the Provider’s eligibility for the PatientFi Growth Fund, available reward options (if any), redemption instructions, and any additional terms and conditions applicable to Diamond Status or the PatientFi Growth Fund. Participation in the PatientFi Growth Fund does not constitute a guarantee of funding, rewards, or marketing investment. All rewards are non-transferable, have no cash value unless expressly stated, and may be subject to additional restrictions or requirements.

Definitions:

Funded Loan Volume: The total net dollar amount of successfully processed, funded, and non-cancelled patient loans originated through PatientFi during a single calendar quarter, calculated per unique practice location.

Loan Transaction: The total net number of successfully processed, funded, and non-cancelled patient loans originated through PatientFi, during a single calendar quarter, calculated per unique practice location.

Net: Refers to the exclusion of any cancelled, refunded, or otherwise voided loans from total funded amounts or transaction counts.

Prohibited Actions: You agree not to take any action which is or may be interpreted to be detrimental to the brand, trademark or interests of PatientFi, including without limitation, attempting to receive rewards in a manner inconsistent with these Terms or the intent of these Terms, attempting to earn rewards through illegitimate channels, participating in fraud, or tampering with the Program.


General Conditions:

By participating in the Program, you agree to release, discharge, indemnify and hold harmless PatientFi, its subsidiaries and affiliates, and each of their respective officers, directors, shareholders, employees and agents (collectively, “Released Parties”) from any liability or damages which may arise out of participation in the Program or out of the acceptance, use, misuse or possession of any Rewards attained through this Program. You also agree that any promotion methods that you use to promote PatientFi products and services to prospective PatientFi customers shall be in accordance with all applicable laws and regulations.

Released Parties are not responsible for late, lost, incomplete, delayed, inaccurate, garbled, undelivered, misdirected Rewards or other errors or problems of any kind relating to or in connection with the Program, whether printing, typographical, technical, computer, network, human, mechanical, electronic or otherwise, including, without limitation, errors or problems which may occur in connection with the administration of the Program, the verification of Providers, the announcement of the Rebates or in any Program-related materials.

PatientFi reserves the right at any time to modify, suspend or cancel the Program or these terms at any time. Any changes PatientFi makes will be effective immediately on notice, which it may give either by posting on the Program Website, PatientFi.com or via e-mail. Your participation in the Program after such notice will be deemed acceptance of such changes. You should review these terms  periodically to ensure familiarity with the most current version. You will always be able to tell when the version was last updated by checking the “Revised Date” at the top of these terms. All questions or disputes regarding the Program, including without limitation, those involving eligibility, participation, fraud and abuse will be resolved by PatientFi.

PatientFi also reserves the right to disqualify and deregister any Provider in the event that PatientFi determines that Provider has violated any of these terms, which determination shall be final. In the event of such disqualification or deregistration, Provider forfeits the rights to receive, and PatientFi is not obligated to honor or pay any Rewards which vest on or after the date of such effective disqualification or deregistration.

Except where prohibited, all issues and questions concerning the construction, validity, interpretation and enforceability of these Rules, or the rights and obligations of the participant and PatientFi in connection with the Program, shall be governed by, and construed in accordance with, the laws of the State of California, without giving effect to any choice of law or conflict of law rules (whether of the State of California, or any other jurisdiction), which would cause the application of the laws of any jurisdiction other than the State of California.

PatientFi Marks:

You acknowledge that PatientFi’s names, logos, trademarks, service marks and other intellectual property (collectively, “PatientFi Marks”) are the exclusive property of PatientFi, Inc., and that you do not have any proprietary rights therein. You may only use and display PatientFi Marks to the extent specifically designated or authorized in advance by PatientFi. Any marketing materials, communications and displays produced by you shall: (1) be produced at your sole cost and expense; (2) comport with reasonable standards of good taste; (3) comply with PatientFi’s then-current marketing and legal compliance guidelines and rules, which may be updated from time to time; and (4) comply with applicable laws. For the avoidance of doubt, all publications or public displays using PatientFi Marks must be reviewed and approved in advance by PatientFi prior to first use. All PatientFi Marks are and will remain the sole property of PatientFi. The permissions to PatientFi Marks are revocable by PatientFi at any time in PatientFi’s sole discretion upon thirty (30) days’ notice and immediately without opportunity to cure if, in our sole discretion, you infringe or improperly use or publish any of our Marks. For avoidance of doubt, all applicable federal, state and local laws and regulations apply.

Warranty Disclaimer:

PATIENTFI MAKES NO EXPRESS OR IMPLIED WARRANTIES OR REPRESENTATIONS WITH RESPECT TO ANY SERVICES OR PRODUCTS CONTEMPLATED BY THIS AGREEMENT. PATIENTFI DISCLAIMS ALL IMPLIED WARRANTIES, INCLUDING, WITHOUT LIMITATION, FITNESS FOR A PARTICULAR PURPOSE, NONINFRINGEMENT, TITLE AND PROVIDERABILITY.

Limitation of Liability:

PATIENTFI’S LIABILITY FOR DIRECT DAMAGES IS LIMITED TO $1,000. NEITHER PATIENTFI, NOR ITS AFFILIATES, SHALL BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, PUNITIVE OR CONSEQUENTIAL DAMAGES, OR ANY LOSS OF REVENUE, EVEN IF PATIENTFI HAS BEEN INFORMED IN ADVANCE OF SUCH DAMAGES OR SUCH DAMAGES COULD HAVE REASONABLY BEEN FORESEEN BY PATIENTFI. THE PARTIES ACKNOWLEDGE AND ACCEPT THE REASONABLENESS OF THE DISCLAIMERS AND LIMITATIONS OF LIABILITY SET FORTH IN THIS PROVISION.
