PatientFi Loan Program 2026 Q1 Natrelle Application Contest Terms and Conditions

Program Overview:
The PatientFi Natrelle Q1 Application Contest (the “Promotion”) is offered by PatientFi, Inc. (“PatientFi”) and provides eligible healthcare provider practices (“Providers”) the opportunity to earn rewards based on PatientFi application volume, application growth, and Natrelle transaction milestone achievements during Q1 (the “Promotion Period”).
The Promotion consists of two components: (i) Regional Application Awards and (ii) Milestone Achievement Rewards.
Eligibility Requirements:
(i) Regional Application Awards:
The Promotion will be administered across three (3) PatientFi sales regions. Within each region, six (6) Provider practices will be selected as winners, for a total of eighteen (18) winners. Providers will be evaluated within comparable application volume ranges (within approximately 10,000 applications), as determined by PatientFi.
- Volume Winners: Three (3) Provider practices per region will be awarded based on the highest total number of PatientFi applications submitted for Natrelle treatments during the Promotion Period.
- Growth / Improvement Winners: Three (3) Provider practices per region will be awarded based on the greatest increase in PatientFi application volume for Natrelle treatments during the Promotion Period compared to Q4 of the prior period.
- Tiebreaker Methodology: In the event of a tie for any Regional Application Award, the winner will be determined based on the earliest date of first application submission during the Promotion Period. If a tie still exists, PatientFi will select the winner in its sole discretion based on overall engagement with PatientFi programs.
Each Regional Application Award winner will receive one (1) Luxury Gift Set with an approximate retail value of $500. Gift set contents are determined by PatientFi in its sole discretion.
(ii) Milestone Achievement Rewards:
All eligible Provider practices may earn milestone-based rewards by completing funded PatientFi transactions for Natrelle treatments during the Promotion Period:
- Providers who complete three (3) Natrelle transactions will receive one (1) team lunch or catered meal.
- Providers who complete five (5) or more Natrelle transactions will receive one (1) premium gift card valued at $350, subject to availability.
Milestone rewards may be earned once per Provider practice during the Promotion Period and are based solely on completed and funded PatientFi transactions.
All rewards earned as part of the Promotion will be fulfilled following the conclusion of the Promotion Period. Luxury Gift Sets will be shipped, and eligible gift cards will be issued, in April 2026. Timing of delivery may vary based on availability and fulfillment logistics.
General Conditions: By participating in this Promotion, you agree to release and hold harmless PatientFi, its subsidiaries and affiliates, and each of their respective officers, directors, shareholders, employees, and agents (collectively, “Released Parties”) from any liability or damages which may arise out of participation in the Promotion or out of the acceptance, use, misuse, or possession of any benefits attained through this Promotion. You further agree not to take any action which is or may be interpreted to be detrimental to the brand, trademark, or interests of PatientFi.
Modification and Termination: PatientFi reserves the right at any time to modify, suspend, or cancel the Promotion or these Terms and Conditions at any time. Any changes PatientFi makes will be effective immediately on notice, which it may give either by posting on the PatientFi website or via email. Your participation in the Promotion after such notice will be deemed acceptance of such changes. You should review these Terms and Conditions periodically to ensure familiarity with the most current version. You will always be able to tell when the version was last updated by checking the “Last Revised” date at the top of these Terms and Conditions.
Limitations of Liability: PatientFi is not responsible for late, lost, incomplete, delayed, inaccurate, garbled, undelivered, misdirected applications, or other errors or problems of any kind relating to or in connection with the Promotion, whether printing, typographical, technical, computer, network, human, mechanical, electronic, or otherwise, including, without limitation, errors or problems which may occur in connection with the administration of the Promotion, the verification of new Providers, or the tracking of transactions.
Governing Law: Except where prohibited, all issues and questions concerning the construction, validity, interpretation, and enforceability of these Terms and Conditions, or the rights and obligations of the participant and PatientFi in connection with the Promotion, shall be governed by, and construed in accordance with, the laws of the State of California, without giving effect to any choice of law or conflict of law rules (whether of the State of California, or any other jurisdiction), which would cause the application of the laws of any jurisdiction other than the State of California.
PatientFi Marks: You acknowledge that PatientFi’s names, logos, trademarks, service marks, and other intellectual property (collectively, “PatientFi Marks”) are the exclusive property of PatientFi, Inc., and that you do not have any proprietary rights therein. You may only use and display PatientFi Marks to the extent specifically designated or authorized in advance by PatientFi. All marketing materials, communications, and displays using PatientFi Marks must comply with PatientFi’s then-current marketing and legal compliance guidelines and rules, which may be updated from time to time, and must be approved in advance by PatientFi prior to first use.
By participating in the Promotion, you agree, affirm, and represent that you have read, understand, and will comply with these Terms and Conditions.
